Transitional diabetes
Our Transitional Diabetes Team comprises of specialist nurses, doctors, dieticians and a psychology service. The team is a new joint working group of professionals from both adult and paediatric diabetes services.
The aim is to provide a more seamless service for our 16-19yr olds with Type 1 or Type 2 diabetes as they transition from paediatric to adult services.
- These clinics run on the first Thursday of the month in the Adult Diabetes Centre. They are consultant led one-stop multi-professional clinics.
There are two clinics one lead by Dr Gupta (Paediatric Consultant) the other by Dr Abedo (Diabetes & Endocrinology Consultant). These clinics are supported by Diabetes Specialist Nurses, Dieticians and a Diabetes Support Worker. Patients are offered appointments every 3-4 months .They are held in the Diabetes Centre.
- The Annual Review Clinics are on the second Monday of every month and are currently held in the Children’s Resource Centre.
The clinic is run by a Paediatric Diabetes Nurse Specialist and a Dietician from adult services.The young people have their blood and urine screened and feet examined. Retinal screening is carried out annually outside the hospital which is monitored by our team. They also have a one to one appointment with the dietician.
Educational support groups are offered. These sessions cover various topics eg: carb counting, pregnancy, alcohol, drugs, smoking, exercise, sick day rules and many more. We hope this will empower our young patient’s to take more responsibility for their short and long term health.
Community Services:
Community services are provided by our Paediatric Diabetes Nurse Specialist: including home, school, college and place of work visits.
