We are delighted to reveal the new DGT 'Long-Term Conditions Passport' (LTC Passport) – this document has been developed to assist communication for patients living with long-term health conditions who frequently use our services.

Within the LTC Passport, patients are able to provide information about their condition, specialist needs/disabilities, current care plan, medical interventions and more. The LTC Passport is owned and by the patient and is taken with them to every scheduled hospital appointment. It is advised that patients store their LTC Passport in an accessible place known to their next of kin or ambulance crew in case of an emergency hospital admission.

The purpose of this is to empower patients to support staff in having a clear overview of their needs which, in turn, enables them to be treated accordingly. The overall aim of the tool is to support a smooth patient journey, increased patient experience and better patient outcomes.

The Passport was designed and co-produced alongside one of our wonderful patients, Suzanne. Suzanne shared her personal experience of attending our hospital and fed back that a communication tool would really help patients and staff to improve continuity of care and patient experience.
