All patients requiring surgery must complete a pre-operative questionnaire before the surgery can go ahead. This will help to assess your health, screen for any long-term conditions requiring optimisation and determine if you are fit to proceed with the operation.
All patients will be asked to complete this questionnaire via a secure web-based portal called Patients Know Best (PKB). This is accessible via the NHS app.
- Completing the questionnaire online allows the pre-operative assessment team to review and decide what tests you may need prior to your operation. Completing it quickly online and answering all the questions may speed up the time to surgery.
- You will receive an email notification when your questionnaire is available to complete. You can opt to complete the assessment online at ease in your home. If you are unable to complete a digital questionnaire then the pre-assessment team will be able to help you during an appointment at a later stage.
Please note that the questionnaire can be completed both via the Patients Know Best (PKB) portal or directly through the NHS App.
Please rest assured that your care will not be negatively impacted if you choose not to complete your questionnaire digitally.
If you would rather complete it in person, then a POA nurse will assist you to complete the questionnaire during your pre-operative appointment.
Patients Know Best (PKB) is an NHS accredited online platform that gives you secure access to your hospital information via the NHS App.
It’s designed to improve your patient experience, access NHS services and hospital information and you can use it to manage your health.
This service is free and available to adult patients (those over 18) and is part of our promise and ongoing commitment to give you more choice and control over your care. It helps us understand the type of care and support you need and want.
PKB is designed to improve your care and experience, help you to be more involved in decisions, get better explanations from us, and give you the tools to manage your care safely at home. Once you have registered with PKB and as we develop the system, you will be able to:
- Access medical letters
- Access online information and resources about your care
- See appointment details
- View admission and discharge information
You can register now on any internet enabled device (smartphones, tablets and desktop computers) using your NHS login on the PKB login page: https://my.patientsknowbest.com
1. Once you are on the PKB log in page https://my.patientsknowbest.com , click ‘Continue to NHS login’
2. You will be directed to the NHS login sign in page
3. Enter your email and click continue
4. Enter your NHS login password & you will be sent a One Time Password (OTP) to your mobile phone.
5. Enter the OTP and click continue
6. You will now need to agree to share your NHS login information with PKB
7. You will be shown a registration form to complete. The form requires you to set up a password and security question for your PKB account, enter your address, and accept both the User Agreement & Privacy Policy for PKB.
8. Complete the form and click next and you will be signed into PKB with your NHS login.
If you do not have an existing NHS login, don’t worry. You will be prompted to create one after entering your email from the NHS login sign in page.

You will receive an email notification prompting you to complete the questionnaire.
Once logged into the PKB portal, you will also be able to access the questionnaire via the Pre-Assessment section.
- You will receive a confirmation email once your healthcare questionnaire has been completed and submitted.
- There is nothing more you need to do at this stage.
- Once the pre-operative assessment team has reviewed your health questionnaire:
- the pre-operative assessment team may contact you for further information or to arrange more investigations; or
- you may be invited for a face-to-face appointment in the pre-operative assessment department.
