Blood Test Appointment Request Form

Please complete form below to request a blood test appointment. Once your form is submitted please allow 3 working days for a response. Please do not contact us until it has been at least 3 working days.

Blood tests for Darent Valley Hospital must be booked at either Darent Valley Hospital (10:00 AM–4:30 PM) or the North Kent Community Diagnostic Centre, Dartford (Monday to Friday 10:00 AM -19:30 PM). Appointments only; no walk-ins. From 2 February 2026, you can book blood tests at North Kent Community Diagnostic Centre during the following times: Monday, Thursday, Friday 10:00 AM -19:30 PM & Tuesday, Wednesday 09:00 AM-19:30 PM.

This service is only for requests from Darent Valley Hospital consultants, clinicians, or departments. GP blood tests are done at Gravesham Community Hospital.

Blood tests at Darent Valley Hospital must be booked – we no longer offer a walk in service. The bookable service is ONLY for requests made by a Darent Valley Hospital consultant, clinician or department. GP blood tests are NOT carried out at Darent Valley Hospital.

We are extremely busy, so we are appreciative of your patience. We endeavour to book your appointment at the requested date and time, however, if we are unable to do so, we will book your blood test in at the next available slot.
