STP Kent and Medway

Transforming health and social care

On the 13 October 2016 NHS social care and public health partners in Kent and Medway launch a vision describing how we are working together and with local communities to transform services and improve people’s health and wellbeing.

The NHS, social care and public health in Kent and We are developing a Health and Social Care Sustainability and Transformation Plan (STP) that will set out how we think services need to change over the next five years to achieve the right care for people for decades to come.

To find out more, please read the covering letter and Transforming health and social care in Kent and Medway, which sets out more about what we want to achieve and our areas of focus. There is a survey for the public Kent and Medway Sustainability and Transformation Plan survey .

All the CCG websites have a set of frequently asked questions and details of ways that local people can get involved and have their say. Over coming months, the websites will also have information about engagement events and other ways that people can make their voices heard.

In the New Year, along with more detailed information about the STP, we will publish a timetable for engaging with the public in Kent and Medway in 2017.

Find out more about what we want to achieve and our ideas so far by reading Transforming health and social care in Kent and Medway T which has details of the local clinical commissioning group websites where you can find out how you can have your say.

You can also join our membership scheme to keep in touch with what is going on and make sure you can have your say at the appropriate time.

Please spare the time to get involved – we need to hear from as many people as possible to get this right for the whole community.

The STP survey on local care, which ran from 13 October to 23 December 2016, has now closed. Thank you to everyone who took part. The results will be analysed and fed into the plan.

If you have any queries, or would like to invite someone to attend a local or community group to talk about the STP, please email Km.stp@nhs.net
